General information
Supporting materials (due in our offices by mail Feb. 10, 2009; no extensions, please ensure materials are in our hands -- not postmarked by -- Feb. 10)GeneracSupporting materials required (in the order shown below) by mail in a three-ring binder, folder, or otherwise held together. If the nominee has not completed anything in a particular area (for example, no papers have been authored), include the corresponding number with “not applicable” in the place of supporting documents. Supporting materials should make it easy for judges to see why your nominee is a standout.

Incomplete nominations will be rejected and will not be considered in this program.

1. Nominee’s resume or curriculum vitae (CV)

2. A brief description (500 to 1,000 words) of the nominee. This should touch on how the nominee stands out in some or all of the following areas:
* Academic training (degrees earned, licensure, etc.)
* Client relations
* Community service
* Dedication
* Innovation
* Inventiveness
* Leadership skills
* "People" skills/mentoring ability
* Personal integrity
* Professional service (with professional societies, organizations, educational events, etc.)
* Public speaking/presentation skills
* Sustained career progress
* Technical skills
* Work/life balance

3. Copies of articles the nominee has authored or co-authored. B&W copies are accepted. Please limit this to no more than 5 recent articles if the nominee is a  frequently published author.

4. A list of presentations/speeches nominee has given. Supporting documentation, such as copies of relevant pages of conference proceedings, should be supplied if available. Please limit this to no more than 5 recent presentations if the nominee is a prolific speaker.

5. Brochures, newspaper clippings, or magazines in which the nominee and his/her professional work or volunteer activities are mentioned. Please limit this to no more than 5 recent samples.

6. Testimonials from clients or others familiar with the nominee's work. Testimonial should be signed or should be on letterhead. Please limit this to no more than 5 recent testimonials.

7. A list of awards received (if this information is already on resume or CV, please note page of resume for us to refer to).

8. Community outreach or volunteer programs. These may be industry-related or not at all related. Please limit this description to 500 words.
